YOUR ROLE

As Thyme Care’s first Complex Care Nurse Practitioner, your pivotal role will revolve around comprehensive case management for members navigating cancer and associated comorbidities. Operating predominantly behind the scenes, you'll collaborate closely with the nursing team, leveraging your clinical expertise to review charting, develop care plans, and provide clinical oversight. Additionally, you'll facilitate weekly case conferences, offering guidance and collaboration opportunities for the nursing team to optimize patient care plans. Your schedule will typically encompass Monday to Friday shifts, with one weekly late shift allocated for urgent care coverage, collectively defined by the team. This role reports to our Nurse Practitioner Lead and VP of Clinical Innovation & Delivery Design.

In this role, you will:

  • Conduct high-touch case management, overseeing approximately 75-80% of clinical aspects for Case Management RNs.
  • Lead weekly case conferences to collaboratively review patient scenarios and refine care plans with the nursing team.
  • Provide clinical input and serve as an escalation point for patient care within designated pods.
  • Review patient information to determine appropriate levels of care upon entry to Thyme Care.
  • Engage in chart reviews, care plan creation, and clinical guidance for oncology-trained nurses.
  • Participate in urgent care shifts as needed, typically limited to 1-2 weekly shifts, without weekend or after-hours responsibilities.
  • Collaborate with the daytime APP leader and three Medical Directors for support and escalation needs.
  • Prioritize patient needs, focusing on triage to prevent hospital admissions and addressing social determinants of health.

  • Qualifications: A Nurse Practitioner with relevant experience, a Bachelor of Science in Nursing, and a compact unrestricted Registered Nurse (RN) license in a compact state. A minimum of 3 years of experience in adult oncology is required. Additionally, possessing certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) is a big plus!

Our salary ranges are based on competitive pay for our size and industry and are part of the total compensation package that includes equity, benefits, and other opportunities at Thyme Care. Individual pay decisions are based on several factors, including qualifications, experience level, skillset, geography, and balancing internal equity relative to other Thyme Care employees. In accordance with New York City law, the base salary for this role, if filled within New York City, is $150,000 - $160,000. The salary range could be lower or higher than this if the role is hired in another location. We also believe that your personal needs and preferences should be considered so we allow some choice between equity and cash.
